Medication Assisted Treatment (MAT) Program and Locations

The the Healthy Living Program is an office based opioid treatment program that utilizes Buprenorphine and Vivitrol to help treat Opioid dependence as well as Alcohol dependence. You can seek these serviced at the following locations:

What is Buprenorphine?
 
Buprenorphine is a pill or film that patients take as a substitute for narcotics (like heroin and oxycontin).

What is Vivitrol? 

Vivitrol® is an injectable medicine used to treat alcohol dependence and help prevent relapse to those who are opioid dependent, after opioid detoxification. 
 
How Do I Join the Program?
 
Any patient who is interested in becoming a part of our program will have to first fill out our MAT Forms Packet and read our Orientation Packet. The Forms and Orientation packet for both Buprenorphine or Vivitrol are available for download at the bottom of this page under attachments, or can be picked up at either the Northampton or Indian Orchard location. When you have completed and signed the Forms and Orientation Packet documentation, please bring them to the Northampton or Indian Orchard location along with your insurance card and an ID.

To join the program you must also be seeing a licensed substance abuse counselor and have insurance referrals if the insurance carrier requires them. To learn more about referrals please see the information under the Insurances, Self-Pay Policy, and Referral Policy option.
